The MEDICAL CARD
All
holders of a current medical card are entitled to a free sight test
with the optometrist of your choice.
You are also entitled to a free pair of spectacles from a designated
range or you will get an allowance towards you frame choice.
If you are eligible
just drop in and we will be happy to fill in an application form for
you.
All you need is your medical card and your P.P.S number.
Normally you will receive authorisation for your free eye test about
a week after submitting this form.
You can then arrange an appointment for your sight test.
Social Welfare
If
you can claim for OPTICAL BENEFIT from the department of Social,
Community and Family Affairs, you are entitled to a sight test and
designated spectacles free of charge.
If you are not working or you don’t pay the full rate PRSI but are
married and your Spouse is paying PRSI, then you are
automatically covered. Just fill in the Spouses Social Welfare form.
Drop in and we will be glad
to help you fill in the necessary form. All you need to bring is
your own or your Spouse’s PPS Number.
